Sliding Fee Discount Program
Our mission of OPTIMUS Health Care, Inc., is to improve the overall health of the communities by providing quality primary, preventive, and supplemental health care services, including health education to the medically underserved, in a culturally sensitive manner with no barriers to health care access.
It is the policy of OPTIMUS Health Care, Inc., to establish a Sliding Fee Discount Program or “SFDP” that includes a schedule of discounts for services, or a Sliding Fee Discount Schedule or “SFDS” that ensures financial barriers are minimized for patients who meet certain eligibility criteria based on income and family size. The Sliding Fee Discount Program does not discriminate on the basis of age, gender, race, sexual orientation, creed, religion, disability, or national origin.
The SFDS shall be applied uniformly, consistently, and equally for all eligible patients. The Sliding Fee Discount Schedule is based on the Department of Health and Human Services’ established Federal Poverty Guidelines in accordance with family size and income. The SFDS shall be revised annually to reflect annual updates to the Federal Poverty Guidelines. Patient eligibility for the SFDP shall be renewed and/or reviewed annually and upon any changes to the individual’s family or income.
