MyChart – Getting Started

OPTIMUS Health Care has a new patient portal called MyChart! MyChart gives you greater functionality and direct access to portions of your electronic health record.

With MyChart you can:

• See your lab results
• View upcoming appointments
• Check medications
• Send messages to your care team and ask questions
• Request appointments

Follow the steps below or download sign up instructions by clicking the links below.

STEP 1: Get an activation code.

OPTIMUS Health Care will send you an activation code by text or email.

STEP 2: Create Your MyChart Account.

BY TEXT

1. For a text, tap the unique link to MyChart. PLEASE NOTE: the link expires in 45 days.
2. Create a username, password, and fill in your information. Make sure your username and password is something you will remember!
3. SUCCESS! You have created your MyChart account!

BY EMAIL

1. From your email, click the unique link to MyChart. PLEASE NOTE: the link expires in 45 days.
2. Create a username and password, and fill in your information. Make sure your username and password is something you will remember!
3. SUCCESS! You have created your MyChart account!

If you received an activation code in a letter or on your printed After Visit Summary, visit mychart.ynhhs.org/MyChart-PRD/accesscheck.asp to complete your sign-up.

STEP 3: Access MyChart from your personal computer or mobile phone.

FROM YOUR COMPUTER

• Visit the MyChart website at https://mychart.ynhhs.org/
• Enter your username and password and click “Log In.”

FROM YOUR MOBILE PHONE

• Go to the app store on your phone and search for “MyChart”
• Install and open the app
• Select “Connecticut”
• Scroll down and select “Yale|MyChart”
• Enter your username and password and click “Log In.”
