| NURSE PRACTITIONER (A.P.R.N.): |
Needed for a pediatric
model practice and for an adult model practice,
to provide direct care ensuring the ongoing
development and delivery of medical services to
patients within Community Health Center practices
located in Bridgeport and Stamford.
REQUIREMENTS: Must be board certified, CT Licensure.

| MEDICAL RECORDS MANAGER: |
Under the supervision
of the Chief Medical Officer will establishes and
monitors systems and procedures for the efficient
maintenance and distribution of medical records.
Under the direction of the IT Director will work
towards the planning, development and implementation
of electronic medical records. Will be responsible
for the supervision and development of Medical Records
Staff throughout all satellites. Must be computer
proficient and experienced with EMR. Certification
required with at least two years experience. Must
be able to travel between sites located in Bridgeport,
Stratford and Stamford. |
| ASSOCIATE MEDICAL DIRECTOR: |
Will provide clinical care and administrative
oversight for patient and health service delivery at
OHC’s Stamford satellites. The AMD will work under
the supervision and direction of the Chief Medical
Officer providing leadership and direction in order
to meet the federally mandated and operational
expectations of Optimus Health Care, Inc. The AMD
will ensure that the structure, mechanisms and
processes are in place at the site to enable the
clinicians and staff to fulfill Optimus mission.
The AMD will be responsible for the coordination of
services with Stamford Hospital’s Residency Program
insuring that its quality care service delivery and
fiscal goals of the Center are met. The AMD must be
licensed in the State of Connecticut, Board Certified
in their respective field of practice and demonstrate
administrative skills and experience to fulfill the
positions requirements. The individual should have a
strong community health and public health orientation. |
| FAMILY PRACTICE PHYSICIAN: |
Needed for Bridgeport and
Stamford location, a Family Model CHC Practice that
provides comprehensive care to pediatric & adult
patients in outpatient setting and inpatient setting
at hospital. Maintain hospital privileges and all
licensing requirements. M.D. degree in Family
Medicine. CT Medical license, residency training
and Board Certified/Board Eligible. |
| CASE MANAGER: |
Successful candidate will provide direct support services
to Center's patients to access various healthcare and
supportive resources in the Stamford area. Collaborates
with community based organizations and agency contacts to
mobilize, support, and maintain patient education and
awareness programs; serves as the facility marketing and
primary contact for local community and government agencies;
assisting in enhancing awareness of and making referrals for
Center's services. Direct involvement in Center’s prenatal
orientation program and other outreach services. Bi-lingual
skills, a BA in social service, health care, or related
degree preferred.EOE

| REGISTERED NURSE AND LICENSED PRACTICAL NURSE: |
For ambulatory primary
care facility. Must be available for extended hours
in the evening and rotation for Saturday a.m.
Bilingual Spanish preferred. Positions available
in Stamford / Bridgeport. |
| DENTAL ASSISTANT: |
Full time - Must be certified
in Radiology. Bilingual Spanish and/or French/Creole
preferred. Experience needed. Position available
in Bridgeport / Stamford |
| FAMILY NURSE PRACTITIONER: |
